We run our own heritage plaques scheme in Hammersmith and Fulham similar to English Heritage’s national scheme. Our first plaque (St Vincent’s Care Home) was unveiled on 20 November 2008. New ones are being installed all the time. In the list below, plaques are arranged by postal district, with full postcodes added to aid location.
